Our Customized Services Include:
Employee Benefit Plan 24/7 Nurse-First Triage Line
Employees want peace of mind that even on a weekend or holiday, they can access a qualified health care professional if they experience a medical issue. For benefit plan participants, our 24/7 Nurse-First Triage Line gives your employees around-the-clock support from registered nurses who use evidence-based clinical protocols to evaluate symptoms and concerns, and who understand the urgency of care needed.
Using a patient-centered approach, nurses will help your employees determine the right level of care, at the right time and in the right setting. In less-critical situations, the nurse can assess an employee’s condition and direct them to the appropriate care, such as home care, a walk-in care location, or a physician who can see them the same day or the next day.
The nurse-first triage line may also include scheduling appointments with providers and/or directing employees to the appropriate care setting such as primary care. If clinically relevant, our nurses can advise employees on recommended care modalities based on their symptoms.
What does this mean for you?
Reduce your employee benefits health care spend, reduce unnecessary ED visits, and leverage primary care as the best point of access.
Bloodborne Pathogen Exposure Nurse Line
Conduit Health Partners works with health systems and hospitals to provide a 24/7 nurse line for employees to call if they experience a bloodborne exposure (BBP) sharp or splash incident. We give your employees peace of mind that should an incident occur, they can easily and immediately report it with one of our registered nurses who will guide them through the necessary protocols.
Our experienced nurses will facilitate timely evaluation and treatment, when indicated, to prevent HIV conversion post-HIV exposure, and to keep the employee at the bedside while the source testing is being facilitated.
In addition to the BBP exposures, exposures to other communicable diseases such as meningitis and pertussis are also coordinated through the nurse line when prophylaxis is needed outside of business hours.
What does this mean for you?
Our experienced clinicians have a proven, effective method of managing a variety of infectious diseases from bloodborne pathogen exposure to other communicable diseases such as meningitis and pertussis. We streamline the process for associates to report their injury and receive prompt testing and medical attention, as well as provide insightful analytics to you as the employer to identify trends that may help reduce such injuries.
